TheCommunity Rehabilitation Service supports individuals in their recovery frommental ill-health. The role of the Support Worker is to work across therehabilitation pathway (based primarily at Tees House), supporting in theassessment of the health, social and wellbeing needs of the individuals usingthe service as well as the delivery of appropriate interventions accordingly.The successful candidate will be able to work both autonomously as well as partof a multidisciplinary team (MDT), attending reviews, MDT meetings and othermeetings whether clinical or not. Main duties of the job This role is focussed around spending timewith our service users and as such it is a privileged role that gives you theopportunity to make a real difference to peoples lives. Therole will support the multi-disciplinary team to delivery individualised carepackages to a small group of service users with both mental health needs and/ora learning difficulties, who are either stepping down from low secure andlocked rehabilitation placements or who are local service users requiringassessment and treatment for their enduring mental ill health.
