We're looking for an experienced Assessment and Intervention Worker to join our dedicated Somerset Talking Therapies Service. Based in Taunton and South Somerset, you will play a key role in delivering high-quality advice, assessments, and Step 1 interventions to patients who are referred to or self-refer to our service. You'll provide consultations not only to patients but also to referrers, including GPs and mental health services, ensuring that individuals receive the appropriate care and support they need.,
* Conducting face-to-face assessments and offering Step 1 interventions to patients
* Consulting with teams across the service and liaising with external stakeholders to ensure seamless patient care.
* Being part of the Duty Team, helping to screen new referrals and maintaining up-to-date knowledge of service offerings to refer patients appropriately.
* Collaborating with secondary care mental health services, leading specialist assessments, and working closely with wider mental health teams.
* Maintaining high standards of electronic patient records and delivering collaborative risk assessments to ensure patient safety.
