Aintree Core 24 and Southport Core 24 are specialist assessment services for service users' mental health who present to the Emergency Department in times of crisis or are referred from acute general hospital wards. The service formulates treatment and risk management plans based on needs assessment with appropriate triage and diversion from the service.
The service operates on a shift system of long days, shorter nine and half hour shifts, and nights to provide 24/7 coverage throughout the year. Recently, the service has undergone Organizational Change to enhance flexibility across the Urgent Care pathway, supporting Aintree, Royal, Warrington Whiston, and Southport Core 24 services when required.
Main duties of the job
To provide high-quality, evidence-based care within the Division for service users and their carers within inpatient and community services.
To work in a socially inclusive way and in line with the recovery model.
To function as the named nurse/care coordinator for service users, undertaking assessment, planning, evaluation, and review of care within a recovery-focused model.
To participate as an active member of the multi-disciplinary team.
To provide advice, expertise, and leadership within the team.
To supervise care for service users presenting with complex and high-risk needs.
To work autonomously, making clinically appropriate decisions based on information received and utilizing the knowledge and experience of the multi-disciplinary team.
To be qualified to act as a mentor for students.

Job responsibilities
To coordinate the care of service users, carrying out assessment, planning, implementation, and evaluation of their care under the guidelines of the care programme approach and under the supervision of the line manager.
To ensure that the service user's care plan is developed to meet the assessed needs of the individual and that unqualified staff are delivering care as identified in the care plan.
To liaise with family members, carers, and significant others when completing assessments and developing care plans.
To undertake and supervise risk assessments and formulate risk management plans for service users under your care, considering contributions from other team members, family members, and carers.
To ensure safeguarding children and adults requirements are followed and participate in meetings as appropriate.
To direct unqualified staff to ensure the delivery of a high standard of care to service users.
To participate in setting standards for care based on sound evidence and research findings.
To participate in the systematic monitoring and evaluation of service users.
To communicate relevant information to other members of the multi-disciplinary team that impacts service users' care.
To utilize verbal and non-verbal communication skills gained through training or experience to impart sensitive information.
