1.Facilitate, promote and maintain a consistently high standard of individualised client care, recognising factors and events that are likely to affect the mental, emotional and physical well-being of clients. Provide advice and support when required. 2. Identify risk and how risks are to be managed in this client group. Inform the Team Manager and other relevant staff where excessive risks are identified. 3. Act upon referrals allocated within the Community Mental Health Team within stipulated response times. 4. Carry out expert holistic assessment of service users and carers in the community setting. 5. Act as Care Co-ordinator and be responsible for ensuring that clients needs and strengths are met through a co-ordinated approach and all clients have their care planned, implemented and regularly reviewed/updated. 6. Maintain a high standard of practice, through the use or a range of psychological approaches, within a health and psycho-social framework. Evaluate approaches and framework regularly through documented research and audits. 7. Carry out the drug administration in accordance with the Trust Policy and the NMC Standards for Administration of Drugs, giving advice, information and support to staff and clients/carers. 8. Maintain independence and to assist clients to perform daily activities for themselves as appropriate. 9. Work collaboratively with the multi-disciplinary team and other agencies eg GPs, District Nurses, carers etc. Ensure that an integrated programme of care is provided in line with Risk Assessment procedures. 10. Develop, educate and establish purposeful individual and group programmes of care, monitoring and evaluating individual clients needs/developments. 11. Act as a specialist resource for information, training and advice to clients/carers and other health care professionals. 12. Organise and participate in the admission, transfer and discharge of clients in accordance with the Trust policies. 13. Implement and/or participate in crisis intervention measures with the client/family or in conjunction with other agencies. 14. Act as a link person for the team with GP practices. 15. Build up a healthy alliance of community networks eg Health promotion, Social Services, Housing, Police and other agencies.
