The Adult Business Unit within Leeds Community Healthcare delivers a portfolio of services that aim to improve outcomes and maximise access to services and their community through integrated working.
The Self-Management service focuses on promoting patient independence, self-efficacy, and health education to improve patient outcomes, increase quality of life, and reduce the use of resources. The service works city-wide, and patients are reviewed at home, in clinics, in hospitals, or in Third Sector services. The team takes a personalised approach, increasing patient knowledge, skills, and confidence to manage their own health effectively by putting in place interventions such as education, health coaching, practice of health tasks, and peer support.
Main duties of the job
The post holder will be responsible for their own diary, ensuring they are working at capacity and supporting colleagues. In doing so, they will set goals, plan, deliver, and evaluate patient-centred care ensuring that they practice within their sphere of competence and knowledge. The post holder will work within standard operating procedures/pathways to manage their own workload independently but with reference to a supervisor/Team Lead within the clinical area. In doing so, they will support the education and independence of patients.
