Job overview
There is an exciting opportunity to join a vibrant community mental health team and support the development of systemic family therapy and family interventions within the Multiple Admissions Pathway Project (MAPP). The MAPP team was created 18 months ago as a pilot project to work with individuals who present with complex mental health needs, are high users of services and at risk of multiple admissions to hospital due to frequent cycles of crisis and difficulties engaging in community support. The team is a multidisciplinary team who work systemically and dialogically. They offer an enhanced level of support that is delivered via weekly open dialogue informed network meetings alongside other services to improve trust and engagement in the community offer of support. They work closely with carers/families and other agencies to ensure a joined up and consistent approach to care. You will be joining a wider Community team where there is a systemic family psychotherapist and an established family therapy clinic.
The project has worked with 11 patients in the last 18 months and there has been significant positive outcomes for all patients. This includes a reduction in bed days, reduction in crisis presentations/episodes and reduction in A & E presentations. Some patients have also been discharged from the pathway. We are now looking to recruit substantively to the MAPP team in Hastings whilst we look to expand across East Sussex in to other ATS's.
