The Education Health Care Plan Coordinator (PO2) plays a vital role in bridging the gap between educational support and healthcare services for students with special needs. Responsibilities
Coordinate the development and implementation of Education Health Care Plans (EHCP) for students with complex needs.
Collaborate with teachers, parents, and health professionals to assess student health requirements and support learning goals.
Monitor and review EHCPs regularly to ensure they meet the evolving needs of students and comply with relevant legislation.
Act as a liaison between schools, health services, and families to facilitate communication and support services.
Provide training and guidance to education staff on the implementation of Health Care Plans and relevant health policies.
Assist in the identification of additional resources and services that may benefit students with special health needs.
Compile and maintain accurate records and documentation related to Health Care Plans and student health services.
Requirements
Bachelor's degree in Education, Health Sciences, Social Work, or a related field is preferred.
Experience working with children or young adults in an educational or health care setting is essential.
Strong knowledge of local educational policies, health care regulations, and special education law.
Excellent communication and interpersonal skills, with the ability to collaborate with diverse groups.
Proficient in using technology and software for documentation, reporting, and communication purposes.
Ability to work independently and manage multiple tasks effectively under deadlines.
Compassionate, patient, and understanding towards students and families facing health challenges.
Hours Per Week: 35.00

End Time: 17:00
Pay Per Hour: £167.01
