The post holder will provide a specialist addictions service and work with service users to comprehensively assess their needs, identify options to meet those needs, and negotiate and review appropriate care within the parameters of ABSDAS eligibility criteria and GP surgeries participating in the GP Shared Care Scheme.
The aim of the service is to support service users with more stable needs who may be prescribed appropriate substitute medication to alleviate withdrawal and cravings, to reduce the use of illicit or non-prescribed drugs until they can achieve a drug-free life, and/or to engage in meaningful psychosocial interventions, continued harm reduction education, and the cessation of any criminal and/or risk-taking behaviour.
The post holder works as part of the multi-disciplinary, multi-agency service, supporting and providing a range of treatment and psychosocial interventions for service users and their families/carers/advocates.
The post holder will work closely with GPs, Practice Nurses, Health Visitors, and District Nurses, and will liaise with hospital wards and units (A&E, Hepatology, etc.). On occasion, the post holder will liaise with the Complex Services Team regarding specific needs (e.g., Maternity services, Community Mental Health Teams (CMHTs), and Criminal Justice services).
Key Responsibilities:
1. Develop agreed Care Plans following the completion of specialist comprehensive assessments, leading to treatment planning, implementation, and evaluation of treatment programmes.
2. Deliver harm reduction information and assess physical, psychological, and social health and wellbeing, along with other nursing/therapeutic interventions, taking action as appropriate and necessary.
3. Plan, implement, and evaluate treatment programmes.
4. Negotiate, develop, implement, and review Care Plans and identify any potential risks with the service user, their families/carers/advocates, and prescribing GP as appropriate.
5. Continually assess physical, psychological, and social health and wellbeing needs, taking action as necessary.
6. Provide treatment option information and education to facilitate informed choice.
7. Participate in the collection of biological samples (e.g., urinalysis) and obtain blood samples, ensuring relevant documentation is completed and appropriate protocols are followed.
8. Support and motivate service users to fully engage with treatment programmes and maintain compliance with medication regimes.
9. Support local GPs within the Shared Care Model, liaise with other support agencies, statutory and voluntary, and facilitate transfers where appropriate.
