Job summary Join our Older Adult Community Mental health Team on the East of the South staff's locality that covers Lichfield /Burntwood/Tamworth and Burton & Uttoxeter as a NMC Registered Band 6 Community Mental Health Nurse. This role offers a unique opportunity to contribute in the effective delivery of high-quality care to service users and their families and you will be responsible for a complex caseload, engaging in therapeutic interventions, following meticulous care plans, and making a positive impact on the lives of those with mental health-related difficulties. The Older Adults Team working closely with internal services and external service providers including GP's, PCN's, Age UK and work closely with social services to ensure social needs are identified and adequately met. The work patterns and client base are unpredictable and there is regular exposure to highly distressing and emotional circumstances. Flexibility is required to ensure the effectiveness of our service and needs of our service users and their carers are met. You will be required to travel over a large geographical area to meet the needs of the service. Main duties of the job As a band 6 Community Mental Health Nurse you will be part of a service that provide specialist input to service users and their carers affected by either a Dementia or mental health diagnosis. Utilising your extensive knowledge and skills in older adult MH and dementia care, conducting assessments, formulating person-centred care plans, and managing risk assessments. As part of this role, you will be expected to: - Undertake screening of referrals, in line with the Service Specification, to identify if referrals are appropriate and to make clinical judgements backed up by clinical reasoning and recommendations regarding appropriate actions/sign posting. - Provide care through the functions of educator and advice giver to relatives, carers and other organisations where appropriate and play a significant role in the supervision of staff. - Ensure where appropriate robust risk assessments and risk management plans are undertaken and that positive risk taking is practiced where appropriate to improve quality of life and maximise occupational performance/independence. - Actively maintain and demonstrate an up-to-date knowledge of national guidelines, standards, legislation and professional developments in current methods in the treatment of client's conditions within the specialism.
