The post available is 0.8WTE (30 hrs/week) with an offered salary of £43,858 (Pro rata).
The development of Integrated Neighbourhood working is supporting Providers to work in a joined-up way, and Primary Care plays a key role in this success. This approach enables more people with health and social care complexities to achieve a greater balance in all-round health.
The role of the Specialist Neighbourhood Nurse is to support both the practice staff and members of the Neighbourhood Team to identify and assist individuals in reducing the risk of unplanned hospital admissions and effectively support those individuals in the community.
Key responsibilities include:
* Being a pro-active member of the Neighbourhood Team.
* Engaging with people deemed to be at high risk of hospital admission.
* Collaborating with in-reach teams to reduce length of stay in hospitals.
* Engaging with people living in care homes.
* Engaging with housebound individuals.
* Being the key contact for patients at the practice.
