Job summary An exciting opportunity has arisen for an experienced Advanced Clinical Practitioner ( Nursing or Physiotherapy ) to work within the community beds ( including Wharfedale Recovery Hub, South and Northwest Recovery Hubs ) This post is to provide exceptional patient care for people within our community beds, to support new ways of working and dynamic thinking across the bed bases. We are looking for an innovative and visionary experienced Advanced Clinical Practitioner who can demonstrate experience and a competency portfolio with transferable skills to a community bed setting. You will need to be a current independent non-medical prescriber who has passed their Masters in Advanced Practice. The post-holder will be proactive in pioneering and developing this role. The Advanced Clinical Practitioner (ACP) will use specialist knowledge and skills to provide healthcare alongside the medical teams to patients. The post holder will act as a role model, delivering advanced practice and promoting clinical effectiveness in care. He / she will be required to work autonomously in delivering independent assessment, clinical examination, diagnosis, clinical decision making and planning of care and treatment, diagnosis, prescribing, health promotion and safe discharge of patients. Main duties of the job Qualifications Registered with relevant professional body Degree-level registration qualification or equivalent post-registration award MSc Advanced Practice including non medical prescribing. Experience Evidence of continuous professional development and the practical application of skills to the workplace. Evidence of significant practical contributions to a service development. Leadership responsibilities. Direct supervision of junior staff and students across professional groups. Contribution to audit programmes. Change Management experience and direct involvement of redesign within a service, including promoting evidence-based practice. Skills/Knowledge Evidence of advanced clinical skills competencies Advanced communication skills and confidence to discuss patient management with other members of the MDT. Confidence and ability to accurately assess and manage complex patient and present to a high professional standard. ln-depth knowledge and experience of community bed bases or inpatient care. Self-motivated and able to work independently and autonomously including partner organisations. Evidence of managing complex patients in stressful and emotional situations where significant events are being communicated and clinically managed.
