A fantastic opportunity has arisen in our Integrated Discharge Team as we are looking for a Discharge Liaison Coordinator to link in with a range of professionals supporting vulnerable adults being discharged from hospital and working in the Transfer of Care Hub (TOCH).
We are looking for an individual who thrives in a fast paced and performance driven environment, that can readily adapt to new challenges on a regular basis. The role is being recruited to permanently, to support with discharges from hospital and working to support at TOCH to prevent unnecessary admissions.
IDT cover a range of areas such as intermediate care and Athorpe (non-acute bed bases) as well as community reviews and supporting same day discharges from the Acute Trust (Rotherham Hospital). As Discharge Liaison Coordinator you work collaboratively with health partners in the Transfer of Care Hub.
The Discharge Liaison Coordinator is vital in supporting Team Managers, Social Workers, nursing and therapy staff who are completing assessments to ensure a timely discharge from hospital. They work very closely with the wards and our health partners to work on the home first approach to ensure where possible patients return back to their home and can maximise independence.
The role of the Discharge Liaison Coordinator involves liaising with patients, families and other professionals to support the discharge process. The role involves supporting with referrals for assistive technology, giving advice and support to patients and their families The role involves using computer systems including LAS and Meditech.
