The starting salary for this role is £60,033 per annum, based on a 36-hour working week.

About the Team
The team evaluates the patient's medical, social, and functional needs within the context of their mental health to determine the appropriate level of care required post-discharge. They collaborate with the patient, their family, and other healthcare professionals to develop a comprehensive discharge plan that addresses the patient's mental health needs.
Additionally, the team coordinates care with community services to ensure a smooth transition. They provide information and support to patients and their families regarding the patient's mental health condition, treatment, and discharge arrangements. Ensuring all necessary arrangements are in place for a safe discharge, including transportation, medication, and follow-up appointments, is also part of their role.
About the role
We are looking for a highly skilled and motivated professional who as the Team Manager will be leading a proactive, dynamic, friendly adult social care county wide team who undertake adult social care assessments and carers assessments using a strength-based approach.
We work collaboratively with our local Mental Health Trust and other stakeholders to facilitate safe and timely discharge from hospital for our residents of Surrey. You will be ensuring front line care services are of a high quality and customer-focused while taking responsibility for leading the team and managing resources.
Your responsibilities will also include putting measures in place for risk management and business continuity, building and sustaining relationships with partner agencies and service providers, and contributing to service improvement initiatives.

In Adult Social Care we value our staff and as part of that commitment we ensure all our staff have a full induction. The induction programme is designed to ensure new starters complete their mandatory training and are equipped with the skills and knowledge to undertake their new role. The programme will consist of a variety of classroom training, E-Learning and workplace shadowing opportunities.
Shortlisting Criteria
To be considered for shortlisting for this position, your application will clearly evidence the following:
* Social work, occupational therapy or other relevant professional qualification is essential for this role.
* Experience of successful management of staff, including managing performance and development while successfully addressing conduct, attendance, and performance capability issues.
* Skill in managing budgets, with substantial experience of managing the delivery and improvement of social care.
* Sound understanding of personalisation and strengths-based assessments will also be vital.
* Significant knowledge and/or experience of health and social care will be essential.
* Direct experience of delivering adult social care services and a comprehensive and applied knowledge of adult social care legislation.
* A full driving licence and have use of a vehicle insured for work/business purposes
Please note, the contractual location for this role is currently Quadrant Court, however it is our intention to move within the local area to Victoria Gate in Central Woking later this year.
