Job summary
George Mackenzie House is a low secure unit providing development opportunity in the Recovery focused care of male and female adults experiencing difficulties associated with a range of acute often long-standing mental health illness's.
The MDT assess and review all aspects of patient care seeking to use a holistic, collaborative approach in the preparation of individual care-plans where each patient is central to this process. We believe in truly collaborative care, support and positive risk taking.
Working on GMH will give the post holder the opportunity to begin the journey of recovery with our patients, helping them develop the skills they require for discharge and then supporting them put these into practise on discharge from the service.
Main duties of the job
The post holder will be an integral member of the George Mackenzie House Low Secure inpatient team but will also have close working relationships with the Forensic Community Teams and others step down services.
This role will be part of a multi-disciplinary team, with a commitment to delivering recovery focused, patient centred care to people with mental health concerns, who have offended or are at risk of offending.
This role will start with the supporting of patients whilst inpatients to prepare for their ultimate discharge from secure services and follow them for an agreed period following discharge to ensure a smooth transition helping to reduce readmission rates to services.

To work within the multi-disciplinary team to support patients transitioning from inpatient secure care into the community or less restrictive environment and maximise their independence. Specifically, in relation to their social support network, housing, finances, and their integration into the community.
To be able to work autonomously; as well as within a team, with the ability to organise and manage own time according to delegated workload.
To ensure effective communication with other agencies, stakeholders, and professionals.
Successfully communicate and report concerns in a speedily manner to senior staff/ management.
To assist and encourage service users in attendance of their physical health as well as their mental health and related activities of daily living.

1. Educated to GCE/GCSE (Maths and English) or equivalent.
