The successful applicant will work as part of the Community Palliative Care Night Team and undertake a broad range of nursing support duties relating to the care of patients with end-of-life care needs in the place where they live, usually their own homes.
They will work under the direction of the Night Palliative Care Team providing care as required to patients with palliative and end-of-life needs and will work within the clearly defined boundaries of their competencies.
Provide care for named patients with end-of-life care needs usually in their own homes in accordance with the patient’s care plan as defined by the Palliative Care Team with minimum direct supervision. The Healthcare Assistant will work effectively and flexibly under direct and indirect supervision to meet the patient and family’s individual needs.

Physical Skills
•Administration of simple medicines following policies and procedures
•Monitor medications and side effects.
•Assists with washing, bathing, showering, hair, and oral care.
•Assists with dressing and undressing.
•Applies lotions and creams appropriately during the provision of skin care.
•Assists with toileting, which may include the use of bedpan, commode, or lavatory
•Undertake simple wound dressings.
•Undertake skin and wound care.
•Undertake foot care.
•Undertake urine analysis.
•Monitoring BPs and BMs
•Collect urine and bowels specimens.
•Pressure Ulcer prevention
•Knowledge of ability to use pressure relieving equipment appropriately.
•Knowledge of and ability to use mobility equipment appropriately.
•Provide basic advice and information to service users on services available to them.
•Assists trained nurse to support patients at end-of-life on the night shift
