Mental Health Services for Older People (MHSOP) in Malton, North Yorkshire are seeking individuals who have an interest in working with Older People in our Inpatient services. This post is open to RMN's, RNLD's and RGN's. Springwood unit is a 14 bedded ward for patients living with Dementia who also have additional frailties, vulnerabilities and behaviours that challenge. Our patients are transferred to us from assessment and treatment wards in North Yorkshire where it has been identified that the patient requires a longer period of treatment in hospital or may have had failed placements in care home environments. The aim of the ward is to continue assessing patients and reviewing and updating treatment plans. Our focus is about positive behaviour support and the development of support plans that then follow patients through discharge into their new homes. We are seeking professional nurses to continue to provide excellent care and support the changing needs of our patients.
Main duties of the job
Attendance at daily MDT report outs and discussions that aid the holistic approach to our care on Springwood Unit and contribute to the effective treatment and management of each patient whilst considering discharge planning throughout their in-patient journey. Taking and receiving handovers and engaging in daily safety review discussions with the nursing team. To allocate tasks to the team, appropriate to their skill level and experience. Contact and engagements with families/carers and liaising with external services. Administration of medication. Risk management in the least restrictive manner and providing a positive approach to managing complex behaviours and involvement with the development of care plans and behaviour support plans.
We will support you by offering regular supervision, training opportunities and encourage your development and role within this evolving service on the delivery of Patient Centred Care, support for carers and evidence based practice.
