Adult Lead Worker

37.5 hours per week
£23,809.50 per annum, subject to pay increase from 1st May
About the role
As a Lead Worker you will be providing early and targeted support to aid the recovery and resilience of single adults or childless couples with multiple vulnerabilities, working flexibly across the city supporting young people in their own accommodation, in locations of their choice and if/when they change accommodation.
You will support vulnerable adults customer group to develop the knowledge and skills they need to better manage their lives, including accessing appropriate services, empowering them to build successful relationships within their community, improving health and well-being and increasing levels of independence as they move through a prevention pathway model.
Key Responsibilities:
Undertake effective needs and risk assessments with people referred to the service in accordance with the service's eligibility criteria.
Develop co-produced, outcome focused support plans that reflect each customer's personal strengths, goals and aspirations.
Deliver a flexible service, increasing or reducing the frequency and intensity of support as required and re-introduce support to dormant cases should any tenancy or support issues arise.
Support customers to sustain accommodation, improve health and well-being outcomes, develop skills required to live independently, including offering practical support, maximising income, budgeting, addressing debt.
Act as a community navigator, linking customers into local services and activities to establish networks of support and reduce social isolation.

About You
We are looking for individuals with relevant experience in providing specialist support that may and will cover a range of activities:
A positive attitude around working with people with multiple vulnerabilities
Experience of agile working across locations to support a case load of customers is desirable but not essential.
Motivated to make a difference and to improve the lives of vulnerable people.
The ability to work both autonomously and as part of a team
Accountable decision making
Ability to maintain confidentiality and work in accordance with GDPR legislation
Reliable and punctual to meet the needs of a diverse caseload of customers across different locations.
Demonstrating a person-centred, flexible, and creative approach to providing support
An understanding of the health and social care needs of the people receiving the service
The ability to develop effective working relationships with colleagues and professionals within partner agencies
