Job summary
The integrated community respiratory team provide a number of services - COPD (including virtual ward), Home oxygen, Pulmonary rehabilitation and respiratory physio.
We are a team of nurses, physios and skills assistant staff. We cover all of NE Essex so access to a car and a driving licence is essential. The service runs 7 days a week so there will be some weekend/Bank holiday working when induction and required competencies have been completed.
We provide clinic based, class based and home based care, with the aim of supporting people to have quality of life and be able to self manage their conditions.
We are seeking an experienced respiratory practitioner to join our team. Ideally you will have experience of community based care. This role includes non medical prescribing so you should have completed (or be near completion) of this qualification.
Main duties of the job
1. To work as part of the integrated team to deliver therapeutic input to patients with the aim of returning them to maximum independent living, within a stimulating environment actively promoting self-management on all activities of daily living.
2. To be responsible for providing highly specialist assessment, treatment and person-centred management for patients requiring respiratory care.
3. Non medical prescriber.
4. To lead and develop delegated aspects of the community respiratory service including planning, co-ordinating, delivering and evaluating the service.
5. To train and develop the team members and other relevant staff members in providing optimum care for people residing in the community.
6. Work with the Managers to provide a safe, effective and high-quality service.
7. Work closely with allied services such as primary care, secondary care, community nursing, specialist nurse and therapy services, palliative care professionals, adult social care, and the voluntary sector to develop care pathways to provide optimum care for people with respiratory needs.
8. Provide day to day leadership of the team in the absence of the clinical lead.
