Frailty and Phlebotomy Care Coordinator NORA PCN
One of the main aims of this role will be to help to support individuals to manage their health and wellbeing in their own homes for as long as possible through the provision of general health care checks and a phlebotomy service.
The Frailty and Phlebotomy Care Coordinators will deliver a phlebotomy service and general healthcare assessments for housebound patients across 3 practices on behalf of North Oxfordshire Rural Alliance (NORA) PCN. This will involve assessing required equipment and consumables from one of our practices, delivering the phlebotomy service and general healthcare checks in the patients' own home and liaising with the patients' GP if there are any health concerns.
Each job-holder will be required to work two days per week with a half day worked alongside the other job holder to complete shared administration and to ensure case-load continuity. The job-holders will be required to share Saturday phlebotomy and frailty assessments work, each working on an alternate Saturday basis.
Main duties of the job
The post holders sharing this role will be expected to work closely with practice teams to coordinate the delivery of phlebotomy and general healthcare services to housebound patients across the PCN.
1. Undertake basic assessments such as taking weight, BP, foot checks, and pulse rate to ensure that any housebound patient seen is receiving sufficient care and to contribute to the management of their long-term care.
2. Deal with a vulnerable group of patients with the potential requirement to highlight those who have limited social/health support and to signpost them to available community services.
3. Learn the roles of the different community health and social care professionals to connect patients to available community services in order to obtain the appropriate care and support they require.
To be successful in this role, you will need to have a strong track record in providing excellent general health care and phlebotomy services. Previous experience of care in the community is not essential but is an advantage. This is a developing role and so there will be the opportunity to shape this role to suit the needs of the community. Previous experience of job-sharing would also be an advantage.

Job responsibilities
1. Using technical equipment to safely draw blood from patients.
2. Explaining the process of drawing blood clearly to a patient.
3. Answering patient questions regarding collection techniques and offering additional resources on the collection process, if the patient requests them.
4. Reassuring and calming patients who might be nervous or scared of the procedure.
5. Ensuring that the patient is as comfortable and relaxed as possible and confirming their agreement to the collection of a blood sample.
6. Checking patient identification against GP/Nurse request and confirming with patient whenever possible.
7. Accurate labelling of samples to confirm patient identity.
8. Applying dressings to the wound created by drawing blood.
9. Sending blood samples to testing laboratories.
10. Ensure that domiciliary blood samples are returned to the hub in time for courier collection and on Saturdays, ensure that domiciliary blood samples are taken to the lab at the Horton Hospital at the end of the shift.
Frailty Healthcare
1. Identify frail vulnerable groups of patients in the community to assess whether there are any unmet health or care needs.
2. Undertake basic assessments such as taking weight, BP, foot checks, and pulse rates to ensure that any housebound patient you are seeing is receiving sufficient care and to contribute to the management of their long-term conditions, eliminating the need for them to visit our practices for support.
3. Perform baseline assessments when appropriate to detect early signs of deterioration in health for housebound patients.
4. When necessary identify urgent needs, taking relevant action or liaison.
5. Notifying the patients' GP if the assessment gives rise to any concern.
6. Refer and liaise with a range of statutory and voluntary sector support agencies, ensuring the person has access to a wide range of support.
7. Provide guidance and support to increase the use of self-care methods.
General
1. Keep timely, accurate, and complete electronic patient records and activity data on EMIS, all of which will be maintained and stored in accordance with professional and organisational standards.
2. To have responsibility for appropriate documentation related to patients in their care to be disseminated to all relevant professionals in a timely manner and in accordance with operating standards.
3. Notifying GP/Nurse of any significant patient issues.
4. During holiday periods you may be required to assist the phlebotomy teams at any of the NORA practices. Be responsible for maintaining Health and Safety standards within the business, in line with the Health and Safety policy.

As a Frailty/Phlebotomist Care Coordinator, you will be required to work to an appointment list for patients in their own homes based across the NORA network area. To do this role a clean UK driving licence and your own vehicle is essential.

Qualifications
* A full, clean UK driver's licence and the use of a fully tax and insured road-worthy vehicle.
