Community Nurse, Builth Wells and Rhayader
The Community Staff Nurse is responsible for the delivery of prescribed nursing care to patients on a day-to-day basis. This includes the holistic assessment of patient care needs and the development, implementation, and evaluation of evidence-based care.
The post holder will be autonomous in their clinical practice while acknowledging their limitations and actively seeking advice as necessary. We are able to support two newly qualified students within our team with a named working mentor, PTHB preceptorship program, and a very supportive team.
Main duties of the job
The post holder will enhance their skills and professional portfolios working across the two community settings, visiting patients in their own homes and facilitating and supporting nursing care on the Health & Social Care In-reach unit. Any successful applicants will be supported by the clinical lead. Your personal development plan will be tailored to your individual needs based on your experience and transferable skills.
We are looking for motivated, enthusiastic individuals to join the Builth Wells and Rhayader District Nursing Team. We welcome new registrants and experienced staff who share our values and want to consolidate new skills and learning within a fully supportive and individualized preceptorship program. We need somebody who is caring, sympathetic, and able to learn new skills with excellent communication skills and an attitude that ensures the highest standard of care for our patients.
Ability to travel between sites in a timely manner is essential.
The ability to speak Welsh is desirable for this post; English and/or Welsh applicants are equally welcome to apply.

Qualifications
* NMC registered
* Diploma modules relevant to Community nursing
* District Nursing Diploma/Certificate or working towards
* Nurse prescriber or working towards
Skills
* Experience in a wide range of clinical skills
* Ability to travel between sites in a timely manner
* Chronic Disease management
* Catheterisation - suprapubic, female and male
* Venepuncture
* ECG
* Administration of IM medication
* PEG/NG management
* Management of central lines
* Administer IV therapy
* Manual handling passport
* Ability to manage a caseload in the absence of caseload holder
* Ability to prioritise workload
* Speak Welsh
Experience
* Experience in a wide range of clinical skills in community nursing
* Community nursing
* To be aware of the needs of the patients and their relatives/carers and have knowledge of voluntary and statutory services
* Able to demonstrate an understanding of current issues, policies, and frameworks influencing delivery of health care in a community setting
* Knowledge of care pathways; Last days of life, falls, and continence
* Demonstrates an understanding and skills in reflective practice
* Knowledge and application of consent policy
* Knowledge of clinical governance and its importance in providing care of a high standard
