Home 1st is a multi-disciplinary integrated intermediate care service, which encompasses collaborative working by health and social care, along with other local providers, to support the physical therapy needs of the people of Milton Keynes. Providing admission avoidance, supported discharge and specialist therapy pathways, this role will require organisational leadership at a dynamic pace.
Main duties of the job
* To provide high quality clinical leadership to the WICU (a 19 bedded intermediate care unit) and the wider Seacole Community Hospital (58 bedded virtual hospital) therapy teams
* To provide transformational, integrated leadership and service modernisation to the pathway
* To enhance the development of evidence-based clinical practice for inpatient rehabilitation
* To provide clear operational leadership to the team to ensure compliance, safety and quality at all times
* To be aware of the national guidelines for inpatient intermediate care and to implement an appropriate strategy within the unit
* To work alongside and in conjunction with the other Home 1 st services to ensure the safety and wellbeing of the patients.
* To work with the service manager and the wider MK system partners to review and develop new pathways of care
* To work with the AHP Professional Leads for CNWL to develop Therapy roles within CNWL and promote the professions

Detailed job description and main responsibilities
* Responsible for managing the Therapy team working in WICU and Seacole Hospital by ensuring that there are safe systems of work that support patient and staff safety, quality of care, effectiveness and compliance.
* Support and guide the operational management of the H1st Inpatient Therapy Team on a day to day basis to ensure the service can meet patient flow in the health and social care system.
* Responsible for ensuring there are robust clinical processes for admission and discharge, management of the patient whilst on the unit and the deteriorating patient.
* To provide clinical and professional leadership to the team.
* To be part of the MDT for the unit, ensuring that patients have the appropriate level of input to optimise their rehabilitation in preparation for returning to their primary place of residence.
* To ensure safe staffing levels for Therapy on the units and manage recruitment and retention to support this.
* To work closely with the GPs, Community Services and the MKUH Integrated Discharge Hub to ensure appropriate and safe admissions to the unit.
* To work with the Home 1 st Community Therapies and Reablement teams, and MKC Social Care to ensure timely supported discharge from the unit to the patient’s primary place of residence.
* Monitor compliance with KPIs and where necessary develop robust action plans to address non-compliance.
* To assess compliance with CQC standards and where necessary develop robust action plans to bring the unit in line to achieve a minimum rating of Good.
* To appropriately represent CNWL and Home 1 st services internally and externally.
* Develop and contribute to the development of care pathways in conjunction with the ICB, MKUFHT and MKC for the management of intermediate care rehabilitation needs.
* To work with the Practice Development Therapist to ensure the teams are mentored in areas of clinical practice and are encouraged and supported to develop clinical and leadership skills.
* To support the team in the implementation of treatment plans to ensure that patients receive a high quality of care at all times, liaising with partnership organisations to risk manage complex cases.
* To promote an integrated working culture across Therapy and Nursing teams to enhance rehabilitation outcomes and the patient experience.
* To be actively involved in the design and implementation of research and/ or clinical trials in order to evaluate that aspect of the service and identify areas for development, maintenance and improvement.
* To develop and maintain good working relationships and liaison with colleagues and other health professionals regarding patient care i.e. GPs, MKC, UCR, UCC and MKUFHT
* To monitor outcome measure data sets to determine service performance and capacity of complexity.
* To lead cross service safety huddles and therapy team huddles to support daily organisational tasks and reporting requirements
* Contribute to the review of patient referrals and provide in-reach liaison with Milton Keynes Hospital to support discharge planning to the inpatient units.
* To work with colleagues to establish clinical protocols and standards to meet NICE guidance and establish an evidence base.
* Undertake evidence-based audit and research projects to further own clinical practice and service development. To interpret and implement changes to national professional policies
* To project a smart, professional image at all times, therefore reflecting the high standards expected within the department.
* To maintain professional standards of conduct according to Trust and other regulating bodies
* To undertake training and CPD to ensure Professional Registration is kept up to date, and to produce certification when requested.
* To attend related courses that would complement the work carried out in the role.
It is the individual's responsibility to hold and update their own training portfolios, in accordance with C.P.D. professional requirements.
* To ensure that all mandatory and job specific training identified by CNWL- MK and the department is kept up to date.
* To monitor and report on KPIs for the different aspects of the service to ensure contracted standards are maintained.
* To maintain safe levels of staffing at all times. To manage recruitment and retention to support this.
* To monitor sickness and annual leave for the team via the organisational sickness absence management policy.
* To monitor competencies, identify training needs and ensure all staff have the skills and training expected for their role.
* To undertake appraisals for the team in line with the organisation policy and assist them in setting and achieving realistic objectives and develop personal training plans.
* To be responsible for organising staff meetings and sessions for clinical supervision.
* To ensure that all staff are trained in the use of equipment and to maintain accurate records.
* To ensure that the service is maintained to the highest standards, equipment is kept in good working order and that faults are promptly acted upon or reported.
* To attend meetings relevant to the post within the organisation and externally as necessary or as designated by the Service Manager.
