Job summary The successful candidate will be a member of the multidisciplinary team, working in a fast pace community urgent care pathway offering a community response to those experiencing a mental health crisis. As an senior mental health practitioner, you will work within a fast paced urgent community care pathway offering an urgent and crisis response to those experiencing mental distress within the Community. Main duties of the job You will conduct initial CPA assessments, agree and develop collaborative care plans underpinned by NICE guidance delivering a range of interventions based upon individual needs. You will be working within West Essex area over a 24 hour period and the assessments and interventions will take place in patients' homes, team bases, hospital and other community settings or via tele-coaching. As required by your line manager, you will have responsibility for the day to day supervision and management of lower band staff within your team. You will be expected to work collaboratively with key partner organisations, patients, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services; providing a 7- day a week service over a twenty four hour period.
