To work as an Inclusion Health Lead Practice Nurse working independently of and in conjunction with other health care professionals to assess, diagnose and treat the conditions of patients, initiating direct referrals as appropriate., Make professionally autonomous decisions for which they are accountable and provide safe, cost effective, evidence based, individualised patient care.
Administer adult and child immunisations and vaccinations in accordance with national and local programmes. Competence in anaphylaxis, resuscitation techniques and other mandatory training as decided by the organisation. Ensure safe storage, rotation and disposal of vaccines and drugs and adequate stock levels are maintained.
Undertake a variety of nursing procedures.
Knowledge and understanding of adult and child safeguarding procedures locally.
Make appropriate referrals to other members of the health and social care teams.
Works in partnership with General Practitioners, Consultants, Nursing and Social Care colleagues to manage patients with a range of acute, non-acute and chronic health conditions.
Refers patients as appropriate to other members of the multidisciplinary team, secondary care and to other statutory and voluntary organisations.
Provides and promotes evidence based care in line with appropriate clinical guidelines and works within clinical protocols, professional guidelines and codes of conduct.
Works within all relevant policies and procedural guidelines.
Review medication for therapeutic effectiveness appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within own scope of practice.
