Job Summary
The role of the Heart Failure Nurse within the community has been established in order to meet the needs of people with heart failure in the Surrey locality. The aim is to optimise the management of people with heart failure, reducing unnecessary hospital admissions and providing a link between the GPs and specialist teams. The post holder will offer education and support to health care professionals, people with heart failure, and those who care for them.

Main Duties of the Job
1. Responsible for communicating highly sensitive information and bad news to patients and carers in an empathetic and supportive manner, empowering the patient to manage their condition.
2. Responsible for teaching patients and their carers to manage their heart failure to the best of their abilities, aiming to prevent short-term and long-term complications, which may be one-to-one or in groups.
3. Communicates effectively with patients and carers to ensure a clear understanding of heart failure and its management, providing and receiving complex, sensitive information.
4. Acts as an advocate for patients, particularly in residential homes, ensuring they receive the understanding and facilities needed to manage their heart failure.
5. Responsible for maintaining and developing effective joint working relationships with nursing and medical colleagues throughout partnership working.
6. Responsible for providing advice and information on heart failure, which may be complex, to medical and nursing colleagues and the general public.
