Alphacare Home Health Agency is a leading provider of compassionate and high-quality home health services. We are seeking an experienced Clinical Manager to oversee our clinical operations, ensure regulatory compliance, and support our team in delivering exceptional patient care.
Job Summary:
The Clinical Manager is responsible for supervising and coordinating clinical staff, ensuring adherence to CMS Conditions of Participation, state regulations, and agency policies. This role involves oversight of patient care services, quality assurance, staff training, and compliance with physician orders. The Clinical Manager plays a critical role in maintaining high standards of patient care, staff development, and operational efficiency.
Key Responsibilities:
Clinical Supervision & Patient Care Oversight:
* Ensure timely, high-quality, and compliant delivery of home health services.
* Supervise nurses, therapists, and other clinical staff, providing guidance and support.
* Review patient assessments, care plans, and physician orders for accuracy and completeness.
* Ensure staff compliance with OASIS documentation, PDGM requirements, and agency protocols.
* Conduct case conferences to review patient progress and update care plans as needed.
Quality Assurance & Compliance:
* Implement and oversee the Quality Assurance Performance Improvement (QAPI) program.
* Conduct chart audits to ensure proper documentation and regulatory compliance.
* Monitor patient outcomes, hospitalizations, and infection control data to identify areas for improvement.
* Ensure compliance with Medicare, Medicaid, and state home health regulations.
Staff Development & Training:
* Provide clinical education, mentoring, and performance evaluations for staff.
* Conduct orientation and ongoing training on documentation, compliance, and best practices.
* Address staff performance issues, ensuring adherence to clinical policies and procedures.
Operational & Administrative Responsibilities:
* Coordinate patient admissions, discharges, and staff assignments to maintain workflow efficiency.
* Collaborate with physicians, referral sources, and case managers to ensure continuity of care.
* Assist in the development and revision of clinical policies and procedures.
* Participate in state surveys, accreditation reviews, and compliance audits.
Qualifications:
* Required: Registered Nurse (RN), Physical Therapist, Occupational Therapist, with an active [State] license.
* Experience: Minimum 3 years of clinical experience in home health, with at least 1 year in a leadership or supervisory role.
* Strong knowledge of CMS Conditions of Participation, OASIS, PDGM, and home health regulations.
