We are in the process of improving our D2A service as ensuring that we actively support the patient flow from hospital in the community, ensuring that patient are being discharged in a timely manner from hospital improving patients health outcomes. You will be part of the larger community therapy team whilst focussing on our D2A service. In your role you will work closely with Occupational Therapists, Assistant practitioners, support staff, clinical assessors ( UCR team), district nurses, specialist nurses and social workers to provide the best possible care to service users in their own homes. Main duties of the job An exciting opportunity has arisen to join our developing integrated community services providing clinical triage, rehabilitation, falls management and support timely discharges from our acute Trust partners. You will work autonomously as well as part of the multi-disciplinary team, liaising closely with health and social care colleagues to coordinate services, promote recovery from illness and enable health, wellbeing and self-care. You will be visiting patients within their own homes using a goal-orientated focus, to maximise patient's independence, safety and quality of life. The post will involve cross site working and community visits.
