To have responsibility for the organisation of a clinical caseload to adults in their own homes and community care settings To be accountable for own professional action and recognise professional boundaries, seeking advice as appropriate To advise head of service on issues of service delivery including clinical activity, service pressures versus service capacity Planning home visits Arranging follow-up appointments Arranging and delivering training to other health professionals within the remit of the service level agreement To develop the service linked with community rehab teams and liaise with community nurses within these groups To work closely with supporting the virtual frailty ward.
