Overview
We are seeking a dedicated Practice Nurse to join our client. The surgery is looking to build a fresh and enthusiastic nursing team that is committed to working alongside our team of GPs, ANPs, and Medical students to provide outstanding patient care.
Our client is looking for applicants with accredited Chronic Disease Management (Respiratory or Diabetes) Qualifications to provide effective Long Term condition management for their large patient list.
Main duties of the job
The Practice Nurse is essential in providing comprehensive nursing care, focusing on patient assessment, treatment, and management within a primary care setting. This role involves a variety of clinical tasks including chronic disease management, minor ailment treatment, immunisations, and health education. The Practice Nurse will work under the guidance and supervision of their team of advanced nurse practitioners, contributing to the practice's goals by delivering high-quality patient care and participating in health promotion initiatives.
The Practice Nurse is expected to perform clinical procedures and manage patient follow-ups, ensuring the delivery of care is in accordance with the latest clinical guidelines and within the scope of practice. This position requires excellent nursing skills, a patient-centred approach to care, and the ability to work effectively within a multi-disciplinary team to improve health outcomes.
Essential Qualifications:
* Previous experience in a primary care setting, with a strong understanding of public health and preventative care measures.
* Competent in using SystmOne.
* Strong organisational skills, with the ability to manage multiple tasks efficiently and prioritize workload effectively.
* Proficient in a range of nursing procedures, including immunisations, chronic disease management, wound care, and health screenings.
* Capable of performing diagnostic tests such as ECGs and spirometry accurately and competently.
