Adult Social Work Services are recruiting for a Team Manager who will be an experienced social work practitioner who will be part of a new fully integrated multidisciplinary leadership team based at East Lothian Community Hospital. Comprising of social workers, community care workers, physiotherapists, occupational therapists, care at home and the third sector this is a new without delay coordinated pathway approach to preventing admissions to hospital and facilitating a more timely discharge from hospital reducing time spent in hospital and improving therapeutic outcomes by delivering more rehabilitative care at home earlier.
You will also oversee the main intake social work system in community adult social work services based at John Muir House in Haddington and ensure harmonisation between both systems facilitating seamless without delay progression of people through this pathway and onwards into locality social work services.
East Lothian Health and Social Care Partnership are adopting approaches that align with the new National Frailty standards and coordination of care via locality-based approaches with close links to the third sector.
This will involve some weekend working so will be required to work across 7 days initially over a 5 day working week but this will be subject to review and likely reviewed once the new pathway has been embedded. This post will also be "onsite" therefore there will be limited scope for home working., To assist in the management of the Statutory Social Work Service providing a responsive and needs led MHO service and providing quality assurance of Adult Protection processes throughout East Lothian in accordance with relevant legislation, national standards, Council policies and plans with a commitment to achieve best value., All preferred candidates for posts carrying out regulated work with these groups will be required to become a PVG Scheme member, or undergo a PVG Scheme update if they are already a member, prior to any formal offer of employment being made by East Lothian Council.

Degree in Social Work (or other recognised Social Work Qualification, i.e. CQSW, Dip SW, CCETSW) or equivalent level of qualification in Occupational Therapy.
Post Graduate Certificate of Advanced Professional Studies (Mental Health Officer
Award) or equivalent
Driving Licence
A full current driving licence is desirable for this post.
