The successful candidate will work within the Wigan Division, and lead on the coordination programme management and operational delivery of the Living Well programme. The role will involve working with key colleagues and partners across Wigan in addition to working with the GMMH Community Transformation Core Team, Innovation Unit and colleagues from the Greater Manchester Health and Social Care Partnership (GMHSCP).
The post holder role will play a key role ensuring the implementation and delivery of the Living Well Programme Milestones to support the achievement of the objectives and be expected to lead on specific work within this programme and support senior leaders and the GMMH Community Transformation Core Team as necessary.
The post holder will develop, project manage and successfully deliver this complex programme, with multiple work streams, taking direct responsibility for the successful delivery of all elements to agreed timescales, within an agreed budget and ensuring safe high-quality services with defined outcomes.

The post holder will take a lead role in development and implementation of the PCN Mental Health Practitioner model.

Experience, Skills
Essential criteria
6. Experience in both health and social care
7. Experience of commissioning, performance managing, contract managing and evaluating health and social care services.
8. Previously responsible for a budget, involved in budget setting and working knowledge of financial processes.
9. Proven experience of successfully managing and delivering complex, high value programmes using a structured programme management process and programme management methods, principles, techniques and tools.
10. Experience of working closely with service users and advocacy schemes
11. Proven skills in the development of alliances and partnership
12. Proven ability in developing complex programme schedules that clearly defines the timeline required to achieve the required outcomes.
