The Hertfordshire Adult Mental Health Community Transformation Programme is implementing new mini teams within Adult Mental Health services to focus support and give more time to care for service users with low intensity psychosis - a new Low Intensity Psychosis Pathway. This Band 6 post will be the named nurse lead for the new mini team in a specified Adult Mental Health Service, working closely with the Clinical Nurse Specialist and the MDT of the service to support this new focus. The Low Intensity Psychosis Pathway Seeks To Promote The Personalised Recovery And Future Care Of Those Service Users Who Are Settled In Mental Health, And Have An Enduring Diagnosis Of Psychosis, By Working In Partnership With People With Psychosis And Their Carers To offer help, treatment, and care in an atmosphere of hope and optimism take time to build supportive and empathic relationships as an essential part of this care.
