Job overview
For entry to the Specialty Doctor grade, applicants must have at least four years of post-registration training, of which at least two years must be in a specialty training programme in General Internal Medicine and/or Geriatric Medicine, or must be able to demonstrate comparable skills and competencies. Evidence of working towards MRCP (UK) or equivalent or already holding such postgraduate qualifications is considered desirable.
The job title of the post is Specialty Doctor in Medicine for Older People, paid on the Specialty Doctor scale. Job Plans may be finalised with post-holders on an individual basis. Depending on experience, training needs, and department requirements, an element of contracted time may be used for CPD, research, or contributions to departmental administration.
The post-holder will be encouraged to advance their career in General Internal Medicine and/or a medical specialty and will be supported in obtaining postgraduate qualifications.
Main duties of the job
You will provide support for the care of medical patients throughout their pathway within the directorate. There will be opportunities to gain experience in specialty outpatient clinics, as well as supporting quality improvement in the department. There are also opportunities for development and progression, particularly for those who wish to develop their careers within Geriatrics and Stroke to a senior level.

Detailed job description and main responsibilities
The postholder is expected to work on the middle-grade tier within the Medicine For Older People wards, providing the relevant expertise and experience necessary to ensure efficient day-to-day clinical care of frail elderly patients throughout their pathway, including within the Emergency Department and Acute Medical Units, as well as within the community. The postholder will be expected to provide supervision to junior members of the team.
The postholder will undertake full clinical duties including daily clinical assessment, diagnosis, treatment, and care planning for medical patients, providing immediate care to deteriorating patients including initial resuscitation where appropriate, making referrals to relevant specialist teams, and ensuring safe discharge and follow-up.
The post-holder will follow the various guidelines and protocols in place in the Division and contribute to these. The post-holder will liaise with GPs to develop appropriate plans for patients within the Intensive Home Support Service.
The post-holder may be expected to attend and manage patients in the Outpatient Clinics under Consultant supervision. Participation in the Division of Medicine Grand Rounds and audit meetings is expected, as well as engaging in available relevant further training and study opportunities.

1. ATTAINMENTS
* Educational Qualifications: Full registration with General Medical Council
* Professional /Occupational Qualifications: MRCP, or evidence of active participation towards MRCP exam
* A higher qualification e.g. M.D. or MSc.
2. TRAINING
* Satisfactory completion of previous posts providing professional training in Stroke care both acute and rehabilitation.
* Experience and training in Thrombolysis and Neurovascular clinic
* Attendance at appropriate professional meetings and courses
* Experience in other areas
* Specialist experience in one or more areas of practice to take a lead role in that area.
3. EXPERIENCE
* Practical experience in management of a wide range of orthogeriatric care/audit/acute medical problems
* Evidence of undergraduate and postgraduate teaching and commitment to these within the Trust
* Understanding of and experience performing audit during general professional and higher specialist training.
* Awareness of basic research methods
* Familiarity with evidence-based practice.
* Attendance at courses in teaching, appraisal, and mentorship
* Previous research projects/publications
