Job Description
As a Social Worker in the Mental Health Team, you will support adults with various mental health challenges, including anxiety, depression, and severe mental illnesses. Your responsibilities include conducting comprehensive assessments to evaluate clients' needs, developing individualized care plans, and facilitating access to mental health services. You will emphasise person-centered support while ensuring compliance with statutory requirements under the Care Act 2014.
Responsibilities include:
1. Conduct thorough assessments of clients to identify mental health needs.
2. Carry out mental capacity assessments in accordance with the Mental Capacity Act 2005. Make applications for Deprivation of Liberty Safeguards (DoLS) where appropriate, ensuring legal compliance and protecting individuals' rights.
3. Respond to emergencies and crises, providing immediate support and intervention.
4. Work with a multidisciplinary team, including psychiatrists, psychologists, nurses, and other healthcare providers, to coordinate comprehensive care.
5. Maintain accurate, clear, and timely records of assessments, interventions, and care plans, ensuring all documentation is in line with council policies and legal standards.
Requirements:
1. Relevant social work qualification.
2. 2 years of experience in adult social work, including assessment and care planning.
