We are looking for an enthusiastic Occupational Therapist who is passionate about rehab to join our team at Farnham Hospital. We work across two wards, each with 21 patients, and are proactive members of the MDT along with our Physios and Assistants, the nursing and medical teams, pharmacy and social care.
You will receive direct line management from the team's B7 OT Lead, who works in partnership with the team's B7 PT Lead. We work collaboratively with the other therapists in the hospital and share learning, IST and clinical skills, knowledge and experience.
We strive for continuous improvement, benefiting both the patients and the team and currently are focusing on refreshing our rehab model whilst preparing for winter.
You will be supported with regular 1:1s and CPD and service development opportunities. You will line manage junior staff and support students on placement.
Main duties of the job
1. To undertake holistic OT assessment, clinical diagnosis and individualised treatment plans for patients on the caseload. These patients may have complex and/or chronic presentations and diagnoses, with varied rehabilitation potential, including palliative patients.
2. Case management, providing expert clinical advice and therapy to patients with a variety of complex conditions.
3. To provide supervision, support and line management to junior staff, and mentorship to students.
4. To work with senior staff to develop a comprehensive service in partnership with Integrated Care Teams and all relevant stakeholders.

Job responsibilities
1. To provide holistic OT assessment, goal setting, individualised treatment programmes, care planning and delivery.
2. Regular review and discharge of patients.
3. To manage a varied caseload, seeking support from senior staff where necessary to ensure the provision of a timely, clinically effective service based on evidence based practice.
4. Complex moving and handling.
5. To be actively involved in, and lead on, projects towards the development & delivery of the service and to contribute to new developments and initiatives.
6. To provide standardised cognitive assessments and treatment programmes as required.
7. To provide comprehensive environmental and functional assessments and treatment programmes, including home visits and access visits.
8. To educate and provide necessary interventions around management of anxiety and fatigue.
9. To provide comprehensive manual handling care plans in a timely manner to ensure the effective and safe delivery of care plans.
10. To identify, assess and manage risk.
11. To triage new referrals, manage handovers and allocate work accordingly.
12. To contribute to interdisciplinary working by teaching and education of professional skills & techniques as defined by an agreed competency framework, both personally and for other staff and students.
13. Where appropriate to take responsibility for the application of new skills to rehabilitation programmes.
14. To provide line management and supervision to junior staff and practice placement students as appropriate.
15. To maintain excellent clinical practice through the CPD process, and taking part in relevant research, audit and development activity as directed.
16. To work closely and jointly at times with the MDT to ensure a timely, holistic and effective care plan is provided to the patient.
17. To be responsible for the day-to-day management and prioritisation of your caseload and be able to respond to urgent referrals and an unpredictable workload. This could require prolonged concentration in an unpredictable environment.
18. To be able to manage potentially stressful, upsetting or emotional situations in an empathetic manner when imparting bad news regarding rehabilitation potential. To ensure this is communicated sensitively to all parties concerned.
19. To keep yourself well at work both physically and emotionally, to access peer, clinical and management support when needed and to be aware of own limitations and escalate accordingly.
20. To report risks and harm to patients using the Trust reporting system.

* Degree or equivalent in Occupational Therapy

* Evidence of experience within physical health setting
* Managerial/supervisory experience and/or training
* Community experience
* Experience of educating and training informal and formal carers
Knowledge
* Knowledge in assessing for and prescribing appropriate equipment required in enabling patients whilst promoting safety
* Ability to assess, plan, and evaluate the rehabilitation needs of client groups and progress to effective outcomes
