It aims to help them as their circumstances change and supports them in positive future choices. About the role As a Family Support Practitioner, you’ll provide a wide range of services across Carmarthenshire, which enables positive outcomes to be achieved. This will be achieved in line with organisational policies, procedures and regulatory requirements. How you’ll create brighter futures Deliver a complex and specialist interventions to women whose children have been fostered or adopted on a 1:1 basis. Provide care planning management, including undertaking assessing, reviewing and managing risks (Carmarthenshire Region) Attending and participating at relevant multi agency meetings, providing a lead role ensuring reflective and analytical practice review Take responsibility for day-to-day resource planning to ensure high quality services are delivered. Establish and developing effective relationships and channels of communication with children, young people, families, colleagues, community organisations, partner agencies and other external agencies Let’s talk about you Willing to travel across region (Carmarthenshire) Experience of chairing, supporting and contributing to service planning meetings.
