As a Specialist Nurse, you will be involved in a wide variety of essential nursing duties, working alongside a multidisciplinary team in a forward-thinking and patient-centered environment. Main duties of the job
1. Provide and maintain a high standard of chronic disease management for patients with chronic diseases, including diagnosis, monitoring, and development of individual management plans, and agreeing these as appropriate with the patient and other health professionals.
2. Provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to the doctors and other members of the primary healthcare team.
3. Complete all tasks within the guidelines of the UKCC Code of Professional Practice and the Scope of Professional Conduct.

Qualifications
* Registered General Nurse.
* Recognised qualification/training in chronic disease(s) (Asthma / COPD/ Diabetes).
* A demonstrable commitment to professional development.
* Keyboard and computer skills including use of clinical computer systems.
* DBS Clearance (the successful applicant will be required to complete this).
* Nurse Prescribing Qualification.
* Experience with System1 clinical system.
* Assessor/Supervisor Qualification.
Experience
* Competent in basic nursing duties required for the post.
* Experience of working in accordance with the NMC Code of Professional Conduct.
* Experience of managing and monitoring the care of Chronic Disease Patients.
* Experience of dealing with the public and patients in a healthcare setting.

£18.92 to £20.16 an hour, hourly rate dependent on experience and CDM specialisms.
