An exciting opportunity has arisen within the Inflammatory Bowel Disease (IBD) Advanced Practice Team for a Band 6 Specialist Nurses. This is a permanent post. The IBD team has recently seen a significant expansion with the addition of a Consultant Nurse and a biologics coordinator.
We are looking for someone with excellent communication skills and a passion and commitment to provide high quality, evidence based care for this group of patients.
The team work across county, managing a busy Advice line service and a large cohort of patients on Biologics. They run Rapid Access and drug counselling clinics and provide support to the Gastroenterology ward and the Medical Day Unit., To assess patients' individual needs; planning, implementing, and evaluating care through the provision of direct clinical care that is evidence-based.
Manage an Adviceline offering support, advice and information
Manage a blood monitoring service for patients on immunosuppressants and biologics
Conduct nurse-led clinics
Provide support for in-patients
Provide support and education to the Gastroenterology ward and Medical Day Unit
