We are looking for enthusiastic Healthcare Support Workers to join our Community Older Persons Mental Health Team which provides care to patients in the community, based at St Mary's Community Campus in Portsmouth.
The Community is a dynamic team and a wonderful environment to learn on the job and develop new skills.
We are looking for individuals with good communication skills along with a flexible and responsive approach as well as a genuine interest in working within this rewarding area of mental health nursing.
As you progress there are opportunities for further professional development, with apprenticeships for both nursing associate and nursing pathways. However, this is based on a further application process. The team are very supportive and will help and guide with any learning need.
The OPMH service is open Monday - Friday 9-5pm.
The team works collaboratively to offer a seamless journey for patients based upon their needs and risks to ensure that care and treatment is assessed and provided with an emphasis on keeping people in their own homes.
Some previous caring experience is desirable though not essential.
Main duties of the job
1. On a daily basis assist qualified staff in the provision of care as detailed in the individual care plans, undertake tasks that facilitate the smooth running of the ward /community as directed by qualified staff
2. Develop rapport with patients and treat all patients, carers and staff with dignity and respect
3. Work with nursing staff, multi-disciplinary teams and other allied health professionals
4. Actively participate in team handovers, providing verbal reports as required
5. Frequently use interpersonal and communication skills to de-escalate and reduce aggressive behaviour if encountered from patients and carers
6. Implement agreed standards of care provision in line with local community and Trust policies, Adhere to all Trust Policies and Procedures
7. Act in a manner to respect the customs, individuality, values, sexuality and spiritual beliefs of all patients
8. Make appropriate objective written entries in patient records, for validation by Qualified staff, to ensure that these are accurate and up to date
9. Using judgement skills when seeking of feedback from patients to inform qualified staff to enable the development of services
10. Be aware of own limitations, seek support if and when the need arises, maintain professional boundaries at all times
11. Periodically collect specimens which may be body fluids, Phlebotomy blood, urine samples
About us
We have been working with other local NHS Trusts to create a new organisation, bringing together all community, mental health and learning disability services from across Hampshire and Isle of Wight. Subject to approvals, the aim is for the new Trust to be formed by July 2024.
Project Fusion is the name for the programme to create the new combined NHS Foundation Trust. Hampshire and Isle of Wight Healthcare NHS Foundation Trust will be bringing the services into a single organisation will result in more consistent care, more equitable access to services irrespective of postcode, and a more sustainable workforce and services. The new organisation will operate locally to ensure services can best meet the needs of different communities.
The new Trust will be comprised of all the services currently provided by Solent NHS Trust and Southern Health NHS Foundation Trust, the community, mental health and learning disability services provided by Isle of Wight NHS Trust and child and adolescent mental health services delivered in parts of Hampshire by Sussex Partnership NHS Foundation Trust.
Colleagues will transfer to the new organisation under Transfer of Undertakings (Protection of Employment) -- known as TUPE on 1st July 2024. TUPE is a legal process to ensure you transfer out of one organisation and into another, maintaining the terms and conditions of your employment.
