Main duties of the job
As an Advanced Clinical Practitioner, you will play a crucial role in supporting our GPs and healthcare team in delivering comprehensive patient care. You will work autonomously, assessing, diagnosing, and managing a range of acute and chronic conditions. Your expertise will contribute to improving patient outcomes and ensuring the smooth delivery of care in the surgery.
Job responsibilities
Key responsibilities:
* Conduct clinical assessments, examinations, and diagnostic tests
* Diagnose and manage acute and chronic conditions in patients
* Prescribe medications and treatment plans in line with your scope of practice
* Provide expert advice, education, and support to patients and families
* Collaborate with GPs and other healthcare professionals to ensure integrated care
* Contribute to improving care pathways and patient experience
Main Duties and Responsibilities
* Ensure that confidentiality is maintained at all times, with a duty not to disclose any confidential information after employment ends.
* Work independently using own initiative and be accountable for own professional actions.
* Assess, diagnose, plan, implement, and evaluate treatment/intervention and care for patients with undifferentiated diagnosis.
* Assess, diagnose, plan, implement, and evaluate treatment/intervention and care for patients with complex needs.
* Proactively identify, diagnose, and manage treatment plans for patients at risk of developing a long-term condition.
* Clinically examine and assess patients' needs from a physiological and psychological perspective and plan clinical care accordingly.
* Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug-based treatment methods into a management plan.
* Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols.
* Work with patients to support compliance with and adherence to prescribed treatments.
* Provide information and advice on prescribed or over-the-counter medication on medication regimens, side effects, and interactions.
* Prioritise health problems and intervene appropriately to assist the patient in complex, urgent, or emergency situations including initiation of effective emergency care.
* Support patients to adopt health promotion strategies to promote healthy lifestyles and apply principles of self-care.
* Assess, identify, and refer patients presenting with mental health needs in accordance with the NSF for mental health.
* Carry out the partners' instructions and requests.
* All staff are required to take their share in providing cover, when necessary, for colleagues who may be absent as a result of holidays or illness.
* Maintain a smart appearance and be helpful wherever possible.

* Proven experience in primary care or community healthcare settings
* Strong clinical assessment and decision-making skills

Qualifications
Essential
* Holds a Postgraduate Degree Qualification in clinical practice (level 7) or equivalent qualification, with evidence of academic study
* Registered Healthcare Practitioner
* Independent prescribing qualification
