Job summary Our practice is looking for an enthusiastic, conscientious, Community Matron/Care Home Nurse. To provide case management to patients with complex needs. Working as part of the Community Support Team based in practice (Senior Community Matron, Care Home Nurse, Care Co-ordinators). We encourage and help with the professional development of staff within the organisation with training as needed.
