We are part of a large integrated drug and alcohol service working with some of Cornwall's most excluded and disadvantaged people. Successful candidates will work as part of the adult community team, alongside in-house services which include outreach, hospital, clinical, criminal justice and a residential rehabilitation facility.

We have an exciting opportunity in our Cornwall services for a full-time Outreach Worker to join our busy and dynamic Hospital Outreach Service based in Truro.
This is an exciting and challenging role that works from a trauma informed perspective based in the community covering all of Cornwall. Due to the outreach basis of this role, a full driver's licence and access to a car is required.
The Hospital Outreach Service provides a rapid response to individuals who are frequent attendees at the Royal Cornwall Hospital in Truro and have issues related to substance use.
The team works within a multi-disciplinary model of practice with partners in safeguarding, housing and homelessness services, domestic abuse services, as well as primary and secondary healthcare and mental health providers.
Essential Skills
* Evidence of continuous professional development & OCN/NVQ level 3 or equivalent or willing to achieve this within 12 months
* Have a full UK driving licence and access to a car
* Demonstrable experience working in a health, social care, or another related setting
* Successful track record of working with substance misuse clients
* Experience and understanding of trauma informed practice
* Ability to manage own caseload
* Understanding and experience of the safeguarding process
* Understanding of the barriers that prevent people with substance use from accessing healthcare
* Have the ability to build and maintain effective relationships with clients and colleagues alike
* Be IT competent
* Good oral and written communication
* Be able to work effectively in a team, supporting others when needed
