We’re searching for an experienced, compassionate, and proactive Senior Nurse specializing in Primary Care and Substance Misuse Services (SMS). Could this be your next career move?

Position: Band 6 Primary Care and SMS Senior Nurse
Location: Exeter
Duration: 3 months
Schedule: Monday - Sunday (07:30pm-21:00pm)
Start Date: ASAP
Pay Rate: £27-£30 p/h
A Typical Day in This Role
As a Band 6 Primary Care and SMS Senior Nurse, you’ll play a vital role in supporting individuals with complex needs, including substance misuse and associated health challenges. Your responsibilities will include:
1. Conducting holistic assessments to understand patients' physical, mental, and social health needs.
2. Developing and implementing care plans tailored to address addiction management, harm reduction, and physical health issues.
3. Providing evidence-based interventions, including medication management and motivational interviewing.
4. Collaborating with multidisciplinary teams, community organizations, and external agencies to ensure cohesive care and support.
5. Acting as a key liaison for patients transitioning between services, ensuring smooth handovers and continued support.
6. Delivering education and advice on harm reduction, health promotion, and relapse prevention strategies.
7. Monitoring and responding to safeguarding issues with urgency and care.
8. Maintaining accurate, timely documentation in line with professional and organizational standards.
What We’re Looking For
Qualifications: Current NMC registration as a Registered Nurse (Adult or Mental Health).
Experience: Substantial experience in primary care, substance misuse, or addiction services.
Skills: Strong communication, advocacy, and leadership capabilities.
Approach: A compassionate, patient-centered approach to care, coupled with the ability to work collaboratively in a fast-paced environment.
Working Hours and Duration
This role offers flexible shifts across Monday to Sunday, providing adaptability to fit your schedule. The initial contract will be for 3 to 6 months, with potential for extension based on performance and service needs.
