The role of social worker within HICTOP aims to provide significant and essential social perspectives which enhance the MDT model through initial assessments for individuals experiencing functional or organic mental health illness. The post holder will be responsible for undertaking person-centred assessments, outcome-based care planning and reviews in line with the Care Act 2015, in conjunction with the the Mental Capacity Act 2005, Making Safeguarding Personal and the NHS 5 year plan. The post holder will work collaboratively in a multi-disciplinary team to provide holistic interventions to service users and carers, including interval services, partner agencies and third sector services. The post holder will be supported to complete their Assessed and Supportive Year in Practice (ASYE) if this required and have access to regular supervision and learning opportunities.
