To provide comprehensive clinical nursing services within Respiratory medicine, with an emphasis on patient centered care and chronic disease management.
Design and conduct regular audit and participation in relevant local and national research
Ensure accurate documentation and maintenance of patients records
Duties include Hospital at Home (H@H) which is a 7 day service and involves rotational weekends and overnight telephone on call.
An exciting opportunity has arisen for an enthusiastic, experienced and innovative nurse who is interested and motivated to develop care and services in all aspects of Respiratory Medicine.
The post holder will work with the existing team members to support the current services, including home care services for patients with respiratory disease. You will be expected to work within the team but also be able to work independently. You will be able to demonstrate appropriate clinical skills and knowledge and plan and organise a clinical case load on a daily basis. You will have excellent interpersonal and communication skills and an ability to foster positive relationships at all levels, including discussion and presentation of relevant information to the multi-disciplinary team. You will be required to promote integration within the multidisciplinary team, all who participate in the care of patients with respiratory conditions.
As a Respiratory nurse specialist you will be required to provide and develop clinical services, participate in research and audit and maintain accurate records and data management as well as data analysis to contribute to service development. You will provide specialist teaching to patients and staff across the Trust and community. You will be required to deliver our Hospital @ Home service on a rotational basis this includes weekend working and overnight telephone on call.
The post holder may be required to work at all sites within the Trust.

Maintain clinical caseload of respiratory patients
Asses, plan and initiate treatment for acutely ill respiratory patients
Assess plan and initiate treatment and services for patients in the community.
Manage nurse led outpatient clinics
Teaching/Education within the team and other organisations
Hospital at Home service
