Learning Disability Liaison Specialist Nurse

Job Purpose:
The Learning Disability and Autism Liaison Services ensures and consistently develops high quality care for people with a Learning Disability (LD) or who are Autistic across the Acute Care Trust, consistent with the principles and objectives contained in the Equality Duty Act (2010), Autism Act (2009), National Monitor framework Standards (Updated August 2015) and towards Learning Disability Improvement Standards for NHS trusts (NHSi, June 2018).
The Learning Disability (LD) and Autism Liaison Services provides cover during the hours of 08:00 to 16:00 Monday to Sunday, but can sometimes arrange to offer support outside those hours by prior agreement. We aim to contact every patient (all ages) with a LD and/or Autism who is admitted to hospital within 24 hours, and work with the patient, carers, ward staff and community staff to ensure that this group of people are able to achieve the same health care outcomes as everybody else.
The Post Holder Will:
1. Act as a positive role model in the field of LD nursing through the provision of effective professional demonstration of competent, accountable clinical and up-to-date practice.
2. Provide advice regarding assessments in relation to needs of people with a LD or who are Autistic - such as use of the mental capacity assessment, safeguarding adults risk assessment, best interest decisions and deprivation of liberties assessments.
3. Ensure appropriate and timely input to care planning for individuals who have a LD or who are Autistic, mainly the assessment of reasonable adjustments needed.
4. Give specialist advice in the management of the interface between hospital departments/wards, specialist community services, adult social care services and primary care advising when appropriate risk assessments and MDT overview is required in complex cases.
5. Develop effective and collaborative working with healthcare and multi-agency colleagues in the management of people with LD or who are Autistic and additional common co-morbidities or other complex health/behavioural needs.
6. Incorporate the philosophy outlined in “Valuing People Now” and the principles embedded within the overarching structures as set within the Learning Disability Strategy/National Guidance, current Trust LD Standard Operation Procedure (SOP), Trust clinical governance framework and Equality Duty Act (2010).
