About the role
To provide early and targeted support to aid the recovery and resilience of people with multiple vulnerabilities, working across a portfolio of longer-term accommodation-based services intended to positively address the ‘revolving door’ nature of shorter term supported accommodation. Demonstrating a commitment to safeguarding in line with policy and procedure you will support customers to develop the knowledge and skills they need to achieve their goals, empowering them to build successful relationships within their community and access the necessary services to develop skills to better manage their health and wellbeing and increase levels of independence as they move through a prevention pathway model.

About You
We are looking for individuals with relevant experience in providing specialist support that may and will cover a range of activities:
8. Excellent communication skills as well as a customer focussed approach are essential
9. Proven ability to work effectively with a wide variety of stakeholders, both statutory and non-statutory
10. Experience of agile working across locations to support a case load of customers
11. Experience in facilitating customer involvement
12. IT Literate and competent with Word, Excel, Support Planning Software and database management and creation
13. Maintaining accurate records that capture identified outcomes and Key Performance Indicators.
14. Ability to manage competing priorities and work to deadline
15. A flexible approach to working within a shift pattern environment
16. An empathetic nature with the passion to support vulnerable young people
