1. Undertake mental health assessments in regard to referrals from the Dual Diagnosis referral hub and PCMHS.
2. Develop a close relationship with Aspire, the Garden House, and primary care colleagues attending regular interface meetings and having informal contact.
3. Provide clinical leadership and expertise to the DDOT and wider system.
4. Triage all referrals and enquiries to DDOT, and allocate cases/interventions accordingly within the team, clinically taking a lead in more complex cases.
5. Maintain an oversight of DDOT patients in the GPN and work with existing MH services to navigate and promote better access to care.
6. Signpost to appropriate services and if required detail out a support plan for the DDOT B4 roles to complete.
7. Support the progress of integrating the DDOT team, providing required reports, statistical and qualitative information alongside PMCHS.
8. Liaise and work in partnership with agencies supporting individuals with housing and substance use issues.
Person specification
Education & Qualifications
Essential criteria
1. Relevant professional qualification e.g., Dip OT, Dip SW, RMN
2. Experience of working in a mental health care setting
3. Experience in the delivery of short term, focused evidence-based intervention
4. Experience of working with mental health service users in a statutory or voluntary capacity
5. Further qualification in mental health related to dual diagnosis or equivalent experience
