In this community nursing role, as a Registered Nurse you will have scheduled visits to care for patients in various settings. You will work with patients of all ages with differing complexity of care needs and play a key role in helping patients remain comfortable in their homes for as long as possible.

What you’ll do:
1. Using appropriate assessment tools/techniques to assess patients’ individualized needs
2. Executing and continuously monitoring care plans
3. Engaging patients using a collaborative approach by building rapport and trust
4. Maintaining and updating professional patient records related to patient encounters and plans
What you bring:
1. Registration with the NSCN as a Registered Nurse in good standing
2. Valid driver’s license and access to a vehicle is required
3. Experience in delivering community care is an asset
4. Reliability and flexibility
5. Willingness to travel to and from patient residences within a designated area
6. Passion for making a difference in your community
7. Required on-call coverage, including after hours to ensure patient safety and to meet patient care needs
