The successful applicant will be responsible for clinical assessments of service users referred into the service through a 'duty system'. Assessments may be conducted initially over the telephone to gather the required information to formulate a plan of care. Dependent on need, there may be a requirement for face to face assessment. The role is primarily focused on establishing an effective 'duty system' but may require care-coordination of service users open to the caseload, dependent on the needs of the service.
The role will involve providing therapeutic interventions and also linking in with other professionals to co-ordinate their interventions, this will all be done within the CPA process.
