An exciting opportunity has arisen for a Senior Community Mental Health Nurse to join an established team at Petersfield and Bordon OPMH.
Responsibilities will include care coordination of service users, Duty Worker on rotation, initial and follow up assessments, and supervision.

The Community Mental Health Team is focused on delivering a recovery service for clients with severe and enduring mental health issues.
Due to the nature of this post, in order to carry out home visits and attend patient meetings, a car driver is essential with access to own car.

Main duties of the job
The Senior Community Mental Health Practitioner will work as a core member of the Community Mental Health team to coordinate the care of a complex caseload of service users, supervising a Band 5 Community Practitioner/Band 3 Community HCSW where appropriate.
Working under the supervision of the Deputy Team Manager/Team Manager, the practitioner will formulate care and risk management plans in accordance with identified need and ensure that these are recovery-focused and developed in collaboration with the service user and where appropriate their relative and carer.
